Odds are this issue is being caused by the web address on your MACS. Under MACS>Update Your MACS Settings, you want to get rid of the slash at the end of your office URL.
For example instead of your web address being this:
http://www.maidpro.com/youroffice/
Make it this:Â
http://www.maidpro.com/youroffice
That slash on the end is often the culprit. In the code for MACS, the system uses your web address and then appends it with "/forms/feedback". Because the backslash is included with the add-on, if you have one on your website in MACS, there are two, so the system can't properly redirect. In short remove the backslash and you shouldn't have any issues. If clients are still commenting that the feedback survey is defective, please log a case and we can look further into it!
